Tourism:

Passport, which should be valid for six months from the date of travel with at least one unused page, along with three photocopies.

Visa application form 509-REF., duly filled

4 recent photographs 2 x 2, front pose (the applicant's name should be written on the reverse side)

Evidence of funds available to support yourself and your accompanying family members during your stay in Dominican Republic. This should be in a form of current bank statement(s), traveler's checks, as well as evidence of your financial interests in your country of residence or citizenship.
Proof of your source of income and if you are an employee a statement from your employer indicating the nature of your employment, salary, and period of vacation leave which you have been granted.

If applicable, a letter of invitation (1 original + 4 photocopies) from relatives or friends that you intend to visit in Dominican Republic.
 
Business:
 
For Business visa, in addition to all the above, the applicant must also provide:
 
An invitation letter (1 original + 4 photocopies) from the business associate that you intend to visit in the Dominican Republic and evidence of your existing ties in Dominican Republic, and the country of which you are a citizen and / or resident.

Medical certificate

Police certificate of good conduct

Business documentations (registration, financial statements, etc.)

For Student visa the applicant must bring all the above listed documents plus the following:
 

Letter from the guarantor or person who will provide financial support addressed to the Foreign Affairs Office, Dominican Republic.

Admission letter from the Dominican University to be attended.

Copy of the registration fee that has been paid to the university.

ENTRY REQUIREMENTS:
n general, foreign citizens must obtain a Dominican visa in order to enter the
country. Visas are classified in Diplomatic, Official, Courtesy, Business, Dependants, Tourism, Residence and Student. The Foreign Service of the Dominican Republic issues these visas abroad thru their Embassies and Consular Offices.

Must travelers must purchase a US$20 tourist card for a stay of up to 60 days. Fees vary for longer stays. A departure tax of US$20 is also collected, although it may be included with the airline ticket.
However, in view of the many agreements that the Dominican Republic has signed with other countries over the years, there are different requirements for different nations.
